Unfortunately, Santa has crashed in front of Aerospace Engineering at the Delft University of Technology while delivering presents. The Elves have asked us to investigate the Crash of Red Sleigh 1. We need to find out the accident cause so Santa can deliver his presents to the rest of the world? Can you help us? Before we get started here is a short introduction on how to conduct an air safety investigation. After an accident, it is common to perform a field investigation, this is where you visit the scene of the accident, collect relevant information which can help determine the cause of the accident. For the investigation, we will try to answer the questions; Who? What? When? Where? Why and How did it happen? In doing an investigation it is important to keep the 4 P’s in mind: People, Paper, Parts and Position. People are the witnesses to the accident. In this case, no witnesses were around when the accident occurred and Santa can’t remember anything of the accident. The second P is Paper which is the documents related to the accident. In the investigation folder, you will find the documents available for this accident. The last two P’s are for the Parts, and Position related to the accident. In this case, it is important to examine the wreckage and surroundings, keeping an eye out which parts are located where and if there are any other irregularities.
